What happens if you type the following at the command line on that same
system?
telnet www.blah.com 80<Return>
GET /index.htm HTTP/1.1<Return>
Host: www.blah.com<Return>
<Return>
Do you get the 404 error right away? Does this index.htm file exist
under your DocumentRoot?
I also noticed something suspicious that I missed before...

Why is htdig 3.1.6 sending an HTTP/1.1 request to your server??? The
3.1.x versions of htdig only support HTTP/1.0? What changes or patches
have you made to your htdig source? What version did you really start
off with?
