You might want to take a look at http://www.htdig.org/FAQ.html#q5.25 and the other FAQs that it mentions. There are a variety of reasons for documents being missed, and the FAQs cover most of them.

words using the search form "open source" (including the quotes) I still
find results containing just the word "open". I want to be able to have a
feature where if the user puts in double quotes or selects an option that
says "EXACT SEARCH" in my form, then I want to search only for the phrase
between quotes.

Phrase searching is only supported in the 3.2 branch, which is not generally recommended for production unless some 3.2 features are an absolute requirement for your task.
