I see this as being one of two things: the URLs are actually slightly
different for each "same" result. e.g.
http://example.com/?blah=one&blah=TWO
http://example.com/?blah=TWO&blah=one

Solve this one by fixing the URLs in the pages so that all links are
identical.

Or for some reason the database didn't purge properly after a small gaffe
somewhere. Solve this one by deleting the database and starting a crawl
from scratch.
