D'oh!!  The code and the documentation disagree on what the default is!
In htdoc/attrs.html, it shows the defaults as <!--htdig-noindex--> and
<!--/htdig-noindex-->, but in htcommon/defaults.cc, they're actually
defined as <!--htdig_noindex--> and <!--/htdig_noindex-->.  Note the
underscore (_) instead of the hyphen (-).  I'd recommend defining them
in your htdig.conf to what you really want them to be, until we get
our story straight.  :-(  I think this was Marjolein's addition, so maybe
she can recommend which it should be.
