Joe, I am not aware of any successful attack on a mainframe system as a result of a software defect.
I am aware of some cases where access was gained to a system by virtue of not changing the default password on a system-supplied userid. I am also aware of some cases where an employee / former employee divulged information permitting an unauthorized person to gain access to a system. John P.
