I'm attempting to order maintenance from Shopzseries for a serverpac I
just installed.

Step 2 of the order process brings me to the following screen:

Step 2 of 5 Report installed software
Service - October 4, 2006, 11:09  
 

Installed software reports are used with service orders to identify the
products and features which are available to be serviced and to exclude
service which you have already received. FMID service critical service
orders require an installed software report. 

 Upload a new report for this order 
 Report name  
 File name  

How do I create the Installed Software Report?

What level of SMP/E is required? We are currently at 3.2.

Note to IBM - a link to documentation describing how to create the
required report would have been appreciated.
