You didn't say what version your OS was or how old the maintenance
was, but I would look to see if the PTF for PQ84939 is applied as Bill
suggested.  Although I would think this would fail 100% of the time
without the PTF.   If you are running z/OS 1.6 or above, then that
APAR can't be the problem.
