Ask me how I know. :-)

Around Y2K time I was upgrading the OS of an acquired company for a
client. IIRC, OS/390 1.2 to OS/390 2.8. I built a clone of their production
system with virtually all system data sets having the same name except the
master catalog (I had to have some place to start and was also using an
SSA). Some data sets were created as new and some were just copied
"as is" to new volumes and then cataloged using define nonvsam etc.

So I copied the production LPAR's couple data sets and cataloged them to
the new volsers. When I IPLed the test system I saw the messages about
the "sysplex in use" etc. We often saw those when we IPLed a test system in
a different "slot" (another available LPAR... there was one test slot per CPC).
So I didn't read the messages carefully and replied "I" to initialize the
sysplex.

Guess which system initialized? (only one guess allowed)

I think that was the last time I crashed a production LPAR. I won't say it was
the only time in my 20+ years of doing this. Luckily it was a very small
environment and my client liked me a lot so they didn't boot me out the door
(I had already been consulting there for about 3 years).

Eventually, I went to work for them full time.

End of story (for now).
