I was told that Format 3 DSCB is for extents 3-16 *in case* of 16-extent
datasets and for 3-123 for "modern" (PDSE, ext-PS, HFS, VSAM) datasets.
I haven't check it in documentation, so I'm not sure of that.

It's true, but the Format-3 DSCB is still limited to 140 bytes, hence
multiples may be needed.
