We hit a problem in this area a while back, and as a result, have to change our 
backup method for OMVS file systems, too, from full volume dump plus logical 
increments to logical ADRDSSU dumps. In addition to that, we had to make sure 
that during system shutdown the F OMVS,shutdown=filesys gets issued, as that 
may or may not have contributed to our problem:

The problem we hit was a mismatch in the number of extents that the HFS has as 
opposed to what the format-4 dscb shows (the dataset actually had 43 extents, 
but only 42 were shown). The 'missing extent data' were all there, and a 
restore of the HFS to one extent 'fixed' the extent problem. The cause of this 
was never found, as we cannot even figure out when we went into that extent.

The thing that cost us the system (several times over, I got to exercise sadump 
again) is now fixed by oa24037, which is hiper. So you may want to install 
that, if you're still using HFSs.
