are you sure the current ISPF configuration is active?

using  ISPCCONF utility you can validate what you think is currently active  -

you need the input that was generated by the ISPCMOD command generally a PDS I think the default member name

is ISPCFIGU

I've used this utility to create an SMP/E usermod

this may be overriding what you think is available

possibly be some local modification here, but I haven't found anything yet.
