At my last site, the Quality Control folks would not accept anything higher than a four on a job they were required to review. This was a result of some product teams bamboozling them on 8s and 12s in the past. Management did not like it when the customers discovered delivery errors. Once QC was required to review system updates as part of new security practices, 4 became the limit for SMPE also.
