I didn't have a policy of regular RSU. But, I would usually bring maintenance 
up to current RSU plus HIPER, IBM , etc. midway between upgrades, usually every 
other Z/OS release. And, I would always bring my running system as close to 
current as was available before starting any upgrade. 

Only once did I try to upgrade without fallback and co-existence fixes on my 
running system. This was long ago, my 1st time and was not a pleasant 
experience.
