Did you POR the machine after you re-installed the last time? If not, then you 
might have a problem in how your IODF is defined. Maybe the IODF was built with 
a new configuration but specified NO in "Switch IOCDS for next POR". Maybe the 
original IOCDS is not sufficient to support the IPL. 

There is something seriously wrong when a zSystem = Windoze.  A full restore 
should never be required because of a power failure. Critically damaging 1 
volume is conceivable but is extremely unlikely. Critically damaging 2 volumes 
seems inconceivable. Losing every logical volume is seems impossible. If the 
business partner suggested you start from scratch, then they should be shot 
before trying to recover.

"ACTZ0182 Activation Complete Load Not Processed" - I'm not sure what this 
truly means but it must be occurring early in the IPL process. I seems to imply 
that the IPL volume is not available or is not IPL'able. 

If your DS6800 has enough space, then allocate some new volumes and install the 
z/VM starter system on new volumes and let it update the IOCDS. IPL (not POR) 
the original system and see if the IOCDS is sufficient. If so, then go into HCD 
and save to the IOCDS. If IPL works, then do a POR to see if it's working 
correctly.

If the original volume was not IPL'able, then maybe the IPL data got damaged. 
You could try updating the IPL data on the RES volume from the starter system.  
  
If in the worst case, you should still be able to re-use existing volumes. 
Hardware support told you the DS6800 is fine. Instead of purging and 
reconfigure, you should be able to retain the current volumes. If you have 
access to the existing user definitions, then you should be able to retain 
existing user volumes.

Thanks for the reply.

The system was reconfigured in early 2012 and, up until three weeks ago, 
running fine. I don't know what all was done accept a new configuration was 
built in the DS6800, 142 or more volumes were configured and zVM was 
reinstalled along with all of the applications. Then I came in and recreated 
all of the user accounts and restored their data from the system backups. 

I too am very skeptical on losing volumes the way we have. 

Now, all of the procedures that you listed above should be something that 
someone from IBM could do, yes? Or is the customer expected to do all of the 
troubleshooting? I am not on site as I mentioned in my first post and since the 
customer is paying for IBM support, I would think that this is something that 
they could or should do. Having said that, I am going to try and get to the 
site if and when IBM shows up because I want to see what goes on. Maybe I will 
be of more help if this ever happens again. 

I just got a note from the customer - after trying to IPL from tape going 
through the load process on the HMC the system and the tape drive both showed 
loading but after about 30 seconds they got the message: The load failed.  
Display the Hardware Manager for the CPC to see detailed messages.   ACTZ01AC.  
I asked him to see if he could find out what the Hardware Manager had to say 
but he has probably gone home for the day.
