What comes to mind is that I have yet to see a single mistake to cause something terrible to happen, it seem to always be a conjunction of events; just a silly example is the program that was leaving behind orphaned storage every time it is executed, it wouldn't be a huge deal if orphaned storage would have been monitored closely and notice that before no more common storage was available.
My advice, try not to leave small problems hanging, they are bound to be just a small part of a bigger problem.
