I've seen similar results when IPLing a 2nd level z/VM system and the page packs were linked R/O.  Sure you have them R/W?

HCPCLS174E using VSE SA util tape

Hello Everyone,

      We have a z890.  z/VM 4.3  and VSE/ESA 2.6.1

We attempting to IPL from the VSE SA utility tape (under z/VM)

I get

MSG HCPCLS174E                                         All Help Information

c) Copyright IBM Corporation 1990, 2002

HCP174E   Paging I/O error; IPL failed

Explanation: An I/O error occurred while the system was attempting to read a

page from a direct access storage device.

System Action: The IPL or DIAGNOSE X'64' failed.

User Response: Reissue the IPL or DIAGNOSE X'64' command. If the failure

persists, notify your system operator.

Programmer Response:  Resave the segment or system that failed. A possible

cause of the condition is a bad DASD pack. If another DASD pack is available,

use it to retry the IPL.  If the problem persists, contact your IBM Support

Center personnel to diagnose and correct the software error.

Can anyone shield some light on this problem?

The z/VM SA DSF/DDR tape works fine.
