In what way was the bolt unexpected? The horse wasn't desensitised. 
Sounds like it had been sensitised to feed bags at least. What else was 
it sensitised to? The horse was a bolt waiting to happen. My ponies 
would have to be willing to have their heads in the feed bag before I 
ride.Desensitisation is a regular part of their training.
 I only just got to see the video on the bitless bridle.( I messed up 
my computer by uninstalling programmes willy nilly) Same scenario 
occuring of pushing the horse way beyond it's comfort zone.  A horse 
that is unhappy in a bridle in a confined space would not be out on the 
road. In fact it wouldn't be in the bridle until it had made friends 
with it through clicker and then on the ground in walk with it on. 
People are in such a hurry to get to a wreck. Then they blame the horse.
OK I know I make mistakes. I'm bound to because I'm newbee. They are my 
mistakes and not the horses. That way I know at least I can find a way 
to fix it ASAP. If I blame the horse how does that serve me or the 
horse. I find it helps not to get emotional when things go wrong. I 
prefer to approach that kind of situation with curiosity. That way 
ideas are more easily forthcoming.
