I wonder if the fact that it was a bitless bridle that has the cross-under 
set up that could have been a problem for the horse.  Sometimes it can 
become a vice for the horse's head, especially if the contact is heavy.  It 
the horse has issues with his poll, teeth, or confinement, it might not be 
the right choice for him.
