There are 2 adapters, both are:
Symbios Logic 53c875 revision 14
(NCR is now called Symbios Logic)
first adapter, serves (0,0,0,0) and (0,0,1,0) devices
second adapter serves (1,0,0,0), (1,0,1,0), (1,0,2,0) and (1,0,3,0)
the driver was chosen according to Compaq's documentation.

id - device on controller
channel and lun are not relevant in that case and must be 0.
Since host 0 (/dev/sda and /dev/sdb ) is not getting any errors, and they use
same driver, I assume that driver is OK.
Problems that remain: bad controller, bad cable on second controller, bad disks,
bad software RAID.
