You take the srpms, add your patches, bump the version number and rebuild the packages. Put them in a separate directory, run createrepo on them which will create a yum repository. Point to your custom repository along with the existing Fedora repository. Run pungi (for regular installation images) or livecd-creator (for live cd/dvd) or use revisor for a graphical wrapper around both.
