Then I tried formatting it, however after a certain point the format fails, because again the access mode changes to read only mode.
I once experienced this issue. Whenever I tried to mount that pendrive, it mounted as read-only because of corrupted filesystem.
A full 'dd' to whipe the whole drive, then making it usable(gparted?) may work.
