The closest I've come is having a few temporary files left over after 
using Photoshop to scan a Q950 motherboard. I tried quite a few times 
to save the pics as .jpg images, but PS crashed several times in a row 
- each time, it left one of its gigantic scratch files sitting in 
Temporary Items. I only lost about 2.5GB when this happened, but for a 
10gb drive that was half full, it was noticeable too.

I could delete the files easily under OSX, but if you're using OS9 it 
may be worth grabbing greg's browser from versiontracker if you don't 
already have it, and peeking around at a few of the hidden 
files/folders across the drive. Turning on List view and Calculate 
Folder Sizes temporarily (even in the normal finder) could locate some 
large files if they're hidden down in the directory tree - even if 
they're hidden, the folders that contain them should show up as being 
pretty bloated.
