Two choices:

Upgrade to the current version of Toast (5.2.1), or at least try a newer
version.

or

Simply remove Toast and it's associated files and re-install it.  It may fix
itself by placing the extensions in the right places.  That worked for me
one time after an OS upgrade.  The OS 9 extensions are usually inside a
folder inside the Toast folder, and if they are, pull them out and drop them
over your system folder and restart.
