I've replaced half a dozen of them. There's a company in Mid-America that had the best price that I could find... Midwest something IIRC. Modem's about $80 and the replace time is about 30 minutes. It lives on top of the logic board. Not a difficult job, just 'fiddley'. You have to remove about fifteen screws.

In my observation, the weakness of the Rev A through C iMacs is the flyback transformer and the later ones have weak modems. On the 350mhz+ iMacs, that's about the only failures I've come across. Typically give some warning before they fail. Slow/dropped connections etc.

Google iMac modem and you'll find what you need.
