Unless you are running the stock OS and nothing more, then you've 
installed *something* :-)

Alas, since you mentioned that it happens so rarely, then it could be 
next to impossible to track the source of the problem. Sometimes you do 
just learn to live with a problem simply because it can't be tracked to 
fix.
