It's Adobe Acrobat Professional, actually... and for complete control of your output, you should convert the original document to a Postscript (.ps) file first, then open in Acrobat Distiller... then set up your parameters.

I think to make custom-sized PDFs, you still need to use Adobe Acrobat itself. The various print-to-PDF options are intended for simple, basic documents.
