ok, first step.  Exchange is at least configured to use ETRN and it's 
arriving at the right ip address.

in the log, note the "SMTP-" process number on the left of the line where 
you find the ETRN commaned (on a busy server, the log is a salad of 
intermixed process numbers as NT multi-tasks the processes) which should be 
followed witht the [ip address] of the Exchange machine. then follow that 
process number from there.
