Jeff,
What do you have in the "Owner" and "Local Administrator" fields ?
Make sure the "Owner" is a full mail address on the domain the list resides
on.
The "Local Admin" is usually just the username of the owner.
Make sure you don't use the name of the list for the "Owner".

Also take a look at the SMTP log and see if the message was actually sent
multiple times.
