I experienced this and there is two major things that caused my errors:

1.  You have to add the "System" & "Creator Owner" accounts from User
Manager to the IMail folder along with the "Everyone" account that's there
by default.  I also added just the "System" account to the actual drive that
IMail is installed on as well.

2.  I had corrupted files in my queue.  Basically, I had aliases that were
resolving to other aliases which went over the 50 number of users mark which
caused the messages to not get sent and caused me the "Access Violations" on
SMTP32.exe.

We have suddenly started experiencing several Dr. Watson errors in
smtp32.exe
