No you were right on your first guess, all was working except the
changepass. The top directory was wrong for the domain.

Craig,

You didn't tell us you just switched to SQL on your post to the other
list; that's an important detail with this problem. Sounds like the SQL
user account you're using doesn't  have read/write permissions on the
database in question. Make SURE you've set up your System DSN for SQL
Server Authentication, NOT Windows NT authentication. Then, in the IMail
Admin console under the External DB configuration fields, you'll need
something like:

  Datasourcename;UID=sqluserid;PWD=sqlpassword
