We have a product called EZSignUp that does that, and more. It's
open-source ASP and HTML, so you can modify it however you wish, and it's
in use on over 500 installations out there. See the link below.
If you know ASP and prefer to roll your own, you'll just need to find a
COM object that will trigger the adduser.exe command line app, and another
COM object or CDONTS to send the welcome message to the new user to
initialize the mailbox.
