We've been experiencing this for months.  Our solution: ditch Imail.
We've put up a unix box to serve mail using sqmail.  So far it's MUCH
better.

I am running Imail 6.06 - 2000.10.13.491 with the latest hot fix. I am
experiencing days when people can get mail but not send mail until I
restart the Imail SMTP service. This condition may occur daily for a few
days then go a few weeks without a restart need. Sometimes two or three
times a day a restart is needed. I am running it on a W2000 server. I
have stopped Windows SMTP. Any ideas?
