The only two that you can safely use are "Relay for Addresses" (where you 
list "safe" IP addresses; anyone coming from other IP addresses needs to 
use SMTP AUTH), or "No mail relay" (which really means that everyone has to 
use SMTP AUTH).

Relay for Addresses is kind of like those cardboard cutouts of police 
officers that some department stores used to use to deter 
shoplifters.  Although they are fairly effective for psychological reasons, 
they don't always work.  With "Relay for Addresses", you're forcing 
spammers to use a return address on your domain in order to get the E-mail 
to go through.  Most spammers have no qualms with doing that.

Yes, just so long as you insert both the "Q*.SMD" and "D*.SMD" files (one Q 
for each D file).
