I have seen this happen in cases where there might be networking delays or
dropped packets... I just had to deal with this scenario myself. There is
probably much more info out there on this, but this might get you started.

Another place I have seen this is in places where Norton AV is installed on
the desktop to scan incoming messages. It can become confused about the
login information and misdeliver same to the server. The fix in this case is
sometimes a reboot, and sometimes re-establishing the server info in OE, and
sometimes more invasive.
