Well, just think of the bad publicity your customer would get, going to 
court to protect the right of spammers to use his mail server for free.  :)

Running an open relay (almost always) means that you are allowing spammers 
to use your mailserver for free, whether you want them to or not.  I know I 
wouldn't pay an ISP to help subsidize spammers, and would have a hard time 
buying something from a company if I knew part of my money was helping 
spammers send spam.

Quite a few people use it, but most people don't prevent the E-mail from 
being delivered (since there are still quite a few open relays out there 
sending legitimate mail).  Some people will add a warning header 
("X-RBL-Warning: This E-mail comes from a mail server that is spam 
friendly", for example) or add a simple "ORBZ:" to the subject of the 
E-mail.  That way, the recipient can decide whether or not to filter on it.

We occasionally get listed in the blacklists because we (intentionally) run 
an open relay (no, spammers can't use it).  That's just a risk we run in 
having an open relay.  We could easily close it, but keep it open to test 
our Declude Hijack, as spammers find our mail server.
