well, I find that extensive blocking in IMGate plus quick and easy 
whitelisting to be effective and manageable.

However, today I am fighting a bitch, aka earthlink, because earthlink is

1) heavily used by spammers forging @sender.domain and

2) earthlink really IS a source of abuse, gets into RBL, and gets harvested 
by my new "intelligent" IMGate scripts for direct bad behavior against an 
IMGate MX.

So, in this case, we have to whitelist the justifiably blacklisted 
earthlink ip's and expose users behind IMGate to abuse from earthlink, 
mixed in with valid mail from earthlink.  Simply no way to distinguish 
between the two reliably.
