Can you gurus try this and see what you get ? I telnet to mail.pakstar.com port 25 (a customer) and get a 421 error (same error as my smtp log shows)
http://www.dnsreport.com/tools/[EMAIL PROTECTED] shows a timeout when connecting to mail.pakstar.com.
Do you have a firewall that might be returning the 421 response?
I call I-mail tech support and they try it and get a connect failed. The mail.pakstar.com host at 208.57.210.46 is offline (unplugged).
Where exactly are you getting a 421 response? If you run telnet, and telnet to port 25 at 208.57.210.46? If the server at that IP is offline, and you are getting a 421 response, there is either something interfering on your end (does your Internet provider play games with SMTP traffic, like a number of large dialup/consumer ISPs do?), or the IP is connecting to another server that is responding with the 421.
