"obviously" you've addressed all The Usual Suspects of mega-sized .mbx files, dedicated disks/disk partitions for spool and mbs storage, purging stale files in spool/ and spool/web, defragging?
can you watch Task Manager mem/mhz/disk while provoking the problem to see if anything spikes?
