I'll second this. We have had the typical problems as well.  We use
Declude and all mail that is flagged as spam is held on the server.  We
wrote scripts that run every 4 hours to parse the held mail and deliver
messages to the users that have mail held.  They are given the option of
releasing a message for normal delivery.  They can release any message
they want without us intervening.  All released messages get logged and
we review the log to fine tune our white list as well as spot other
problems.  We ran into the problem that everyone seems to have
conflicting definitions of what SPAM really is and people wanted to have
yahoo.com and hotmail.com white listed.
