This has happened to me before. I had Imail using our ISP's DNS for lookups
and I think there was a latency issue because it was returning wrong Ips for
domains. I set up DNS on the Imail box itself (Win2K server with plenty of
resources to do so) and the problem went away immediately.  

Try using DNS either on the Imail box or as close/fast as possible.

The other cases are probably different -- this is a very, very strange one.
IMail is finding an IP address that doesn't belong to the domain that it is
sending to.  I have never seen it do that before.  I would recommend
upgrading to the latest version of IMail.  If you are on IMail v8, you may
want to try disabling the DNS caching.

Also, you can manually check your DNS server to see whether it is reporting
the correct answer.
