One domain, spam filtering only set to alter the subject with [Spam]. 

No forwarding to a spam box, users set up local rule to filter in Outlook.

Are your anti spam setup on the server to forward to a mailbox? We use
filters to catch the X-Header options so we can be as granular as
possible for all our domains.
