A quick follow-up to this. I upgraded the server from IMail 7.15 to IMail 8.12 and it didn't make any difference to the CPU load, which ruled out the IMail software itself. A complete overhaul was necessary, so I installed Win2k3 on new hardware and migrated IMail over; success - it hardly makes a dent on CPU load now, and we're even back down to one CPU instead of 2.

I think it must have been something OS-related, as the hardware is identical. I'm going to pull the drives out and put them back in the original hardware at our hosting center once I'm happy that no new issues have been found, that'll tell us for sure.
