John, Are you sure it is vacation and not Info Manager? If you send 2 times to the user, do you get 2 replies? If so, it is more likely to be Info Manager.
Look for other files in that users directory. Ones with .ima extensions. If vacations files were deleted, are they back? If not after your 2 email attempts (vacation.snt should have been created if vacation is enabled), then once again that points to Info Manager and its files.
