We, too, have medical clients who occasionally send legitimate ZIP files.
After Ipswitch came out with the ability to STRIP or REPLACE attachments, we
decided to block all binary attachments except ZIP files by replacing the
files with text.  Within a week, we had experienced more than 1,500
self-extracting ZIP files that were noting more than viruses attempting to
propagate themselves via e-mail.

Upon experiencing that kind of attack, we added the ZIP files to the to the
replacement utility and, within 1 week, had stripped more than 7,000
infected ZIP files from incoming messages.  We sent a broadcast message out
to all of our customers and not one of them had a problem.  Most had
previously been the recipient of an infected ZIP file (caught by an
anti-virus program in every case).  Not one complained.

Given the current climate of viruses and hackers, we'll continue to block
all binary attachments for the foreseeable future, including ZIP files.

Yes, but what happens to the attachment when it is stripped or replaced - is
it just discarded, or is it still available somewhere in case it was a good
attachment (some of our customers tend to send self-extracting zip files).
