Mohamed, It is not clear what these concepts are. Is your form to document physical exam or review of systems? If so, then you need to be clear about:
1) Why are you collecting this information? (In my experience, it is almost never of clinical use, so you have to have a real reason for making clinicians enter it) 2) Be sure what the concepts actually mean. What is being examined/reviewed? Do the categories make sense? What does ABNORMAL/NORMAL mean in your context? Looking at what other people have created in the dictionary (for example CIEL/MVP or PIH) will help you make sure you are not creating something that doesn't make sense. 3) Search an existing concept dictionary (maternalconceptlab.com/search.php). Look for thinks like 'card exam' or 'review of systems'. 4) If you don't have them in your dictionary, you can use maternal concept lab to export them and then import them into your dictionary using metadatasharing module. 5) If you want to short cut this, either use 1.9 with the CIEL/MVP dictionary or you can download a 1.6.x version from dropbox.
