Thanks for the info. I'm not sure if that error is meaningful or a red herring. Similar errors have been reported before, but I'm not sure they have had this impact:

My best guess is still that this is a javascript error and has been introduced by one of the modules you are running. I would try stopping modules, seeing if the behavior is fixed with no modules running, and then add them back in gradually until you identify whether one in particular is the root cause. My hunch is that one of the following are the likely cause, as they likely modify part of the patient dashboard:

Patient Image Module
Custom Branding Module
Household Module
Growth Chart Module

** You can see that I was able to un-enroll the patient from the "Maternity Care Program" in November 2011. (therefore the pop-up box was working)
** When I click on "Child Health Program", no pop-up appears
** When I hover over "Child Health Program", I see "javascript:showEditPatientProgramPopup(7)
