Every report I've had of that so far has turned out to be operator error:
some other filter trapped the mail and put it into Trash. Next time you see
it happen, check the TF log and the SCSMFilter log to see exactly what
happened: whether TF marked the message as "Pass" (if you have headers
turned on, you should see a X-Trash-Finder: line to that effect), whether TF
filtered the message, or whether some other plugin filtered the message
(including SCSMFilter itself).

There's always the possibility of a bug, of course, but this is an unlikely
one given the way the code works (once the message finds that it is a Pass,
it immediately returns and stops filtering).
