As for it being a dial-up line -- it could be, but I'm not sure because I
don't know which client caused the error.  

After your explanation of what causes the FINAL MSG RENAME FAILED message, I
was able to find out it is happening because McAfee is deleting the .msg.tmp
file because it found a virus in it. I guess I can just ignore those
messages.

Yes, this is the new error logging.  Anything that hasn't been investigated
gets written to the log.

Something at the tcp/ip level caused the connection to break.

Is there any chance that the connected ip address is a dialup line that
dropped out or had line noise?

SMTPRCV writes the file out as Bxxxxxxxxx.msg.tmp to get the file from
memory to disk.  It does this to prevent the delivery service doing anything
with it until it's ready.  Then it renames the .msg.tmp file to the .msg
(same thing happens with the .rcp file).  

In this case, it threw an exception "file access denied" when it tried to
rename the msg file.  Winnt error code 5.  I'll do some experiments to try
and find what causes it (maybe the .msg file already exists, or there is
still some sort of lock on the .msg.tmp file).  The ms kb goes on about
permissions, but I doubt this is the case.
