Appears a long message ID was in fact the culprit.
Do you have any automation to the removal of said messages or simply
watch for the effects?

I have a couple of IMS servers that are front-end mail filters for 
Exchange 2003 boxes and the only problem I have seen is that some 
messages created in Outlook on outside PCs and sent in will have really 
long message-IDs, maybe 300 characters or so, and Exchange will not 
accept these and then mail piles up behind that one message. That has 
happened several times.  But I can't think of anything else that I have 
had major problems with.  Also, I am not running Exch SP2 anywhere right

now, so I cannot speak for how that might change things.
