Are you sure you built the kernel with the debugging enabled (that is, the
EXCLUDE: idbg edited to INCLUDE: idbg along with following the instructions
at the end of the /var/sysgen/system/irix.sm file)?
It's been my experience that you must build the kernel with debugging enabled
to allow dynamic AFS loading, but that debugging does not need to be enabled
if you build the kernel with the AFS extensions already loaded.
